Total transformation of care and support: Creating the five year forward view for social care
The Five Year Forward View sets out a case for upfront investment in the NHS to transform it into a service which reaches people in their homes and communities with early, effective interventions and builds partnerships with people with longterm health conditions and their families. It is widely accepted that this will only succeed where
social care makes the same transformation. Social care has a track record of transforming services, such as delivering personal budgets, asset-based approaches and co-production. Both systems share similar goals. It is now time to bring health and care transformation together.
This paper explores the potential for scaling up the most promising examples of care, support and community health services, initially using data from Birmingham City Council, modelling their outcomes and costs. Originally published in November 2016, it has been updated to include additional models.
