Living good lives: Effective hospital discharge planning for adults with learning disabilities and/or autistic adults to prevent future admissions

This is the third of three webinars in a series aimed at supporting health and social care practitioners to enable adults with learning disabilities and/or autistic adults to live good lives along the homes not hospitals pathway

  • How to access resources and training to support positive hospital discharge and prevention of readmission for adults with learning disabilities and/or autistic adults in hospitals
  • What good hospital discharge planning looks like, including contingency planning, MDT support, care and treatment reviews, and taking a life-span approach.
